Steps Required:

1. Meet with Facilities Maintenance Supervisor to discuss your selection and the preferred location tbuchanan@loyalist.ca 613-389-3648 ext.204
2. Pay for the item at the Main Office of the W.J. Henderson Recreation Centre, 322 Amherst Drive, Amherstview 613-389-3648.
3. Order the plaque from Centennial Engravers, 2698 Princess Street, Kingston 613-389-0180.
4. Provide plaque to Facilities Maintenance Supervisor
5. If a tree is being purchased, select the tree from stock at Potter's Nurseries, 690 Golden Mile Road, Kingston 613-389-3333
6. An official tax receipt will be issued by Loyalist Township

Tree
Cost: $500
Includes installation, soil/mulch/tree skates, and the block for the 4" x 4" plaque*. Choose from Autumn Blaze, Crimson King, Emerald Queen, Sugar Maple, Red Oak, Colorado Spruce, Colorado Blue Spruce.  Trees are a minimum 2.5" caliper.

Park Bench
Cost: $300
Includes the bench and installation*. Plaque size 2"x7". We are no longer able to accept bench donations at Fairfield Park at this time due to the large number of benches currently located in the park.

Picnic Table
Cost: $300
Includes the picnic table and installation*. Plaque size 2"x7"
